# Different clinical roles carry different authority

A nurse practitioner can diagnose illness under a doctor agreement; a physician assistant can also take part in care. The South Carolina questions concern the person’s license, authorized work and connection with a physician.

Identify the route and the person responsible.

## The myth that every clinical title means the same thing

When several people take part in a visit, it is easy to lose track of who makes the decision. Ask each person to set out their role in ordinary words. Who takes the history? Who decides if a prescription fits? Who reviews a change in the plan? These are questions about the care being talked about, a way to understand the work being done.

As checked November 3, 2026, the [South Carolina nursing board](https://www.llr.sc.gov/nurse/) licenses nurses and advanced practice registered nurses. Nurse practitioners are part of the latter group. An RN, or registered nurse, has a different license from an advanced practice nurse. The title nurse alone does not settle permission to prescribe.

## A nurse practitioner’s agreement has a purpose

For medical work such as diagnosing illness, a South Carolina nurse practitioner works under a written agreement signed with a doctor. That doctor needs to be available for advice. The [nursing code](https://www.scstatehouse.gov/code/t40c033.php) , checked November 3, 2026, says the agreement sets out treatments, drugs and cases that need doctor review. The doctor need not sit beside the nurse at each visit.

Ask how that arrangement works for the concern being talked about. If a question needs the physician’s input, who makes that connection? How does the reply reach you? The answer can be practical and brief. It need not involve reading the entire agreement during the visit.

### Prescribing is a separate permission

On November 3, 2026, South Carolina required nursing-board authorization for a nurse practitioner to prescribe. Prescriptions must remain within the person’s specialty and practice agreement. The [state rules](https://www.scstatehouse.gov/code/t40c033.php) add limits for drugs with extra abuse controls. A nursing title is not a promise that any requested medicine falls within that authority.

Ask who will sign a planned prescription and what role that person is using. If a different person is named after the first conversation about your details and before the medical visit, ask for that person’s name and role. Keeping this clear makes it easier to direct later questions to the person in charge of the plan.

## A PA has a supervising doctor

A South Carolina physician assistant (PA) works within written rules for allowed tasks under a supervising physician, under state law, reviewed on November 3, 2026. The physician need not always be physically present. [The PA rules](https://www.scstatehouse.gov/code/t40c047.php) allow prescribing only as part of that assistant’s authorized work. A PA’s permission does not show what a nurse or another professional is allowed to do.

Ask which tasks the PA handles and what happens when a concern falls outside the allowed tasks. An answer about supervision is useful when it explains the path a question takes. It is less useful when it is only a name with no way to understand the next step.

### Remote care keeps these boundaries

The November 3, 2026 state record permits nurse practitioners and PAs to use telehealth while keeping their license, agreement and limits on work. [Remote care rules](https://www.scstatehouse.gov/code/t40c033.php) do not remove those limits. Ask about the same roles if the talk happens in a room or through a screen.

Bring the role details into a broader comparison of care routes. Consider how the check works, how results are talked about and how follow-up reaches the person in charge of care. There is no need to infer the entire plan from a title. A clear account of who does what is a better basis for the next talk.
